You may have received a text message from us asking you to have your blood pressure checked This is because:

• The NHS recommends that everybody over 45yrs of age should have their blood pressure checked at least once every 5 years. 
• or beacuse you have Hypertension and a review of your blood pressure is due but no blood test required.

If you wish to have this  done you can come to the surgery and use the self monitor in the waiting room. No appointment needed.

Alternatively you can send us a home reading if you have a BP machine at home. Please e-mail your home readings to petworth.surgery@nhs.net 

If you do not want your BP to be checked please use the below form to Decline. If you decline we will not send you an other invite for one year, but you can change your mind anytime.
