If you received a text message asking you to visit this page, it is because you are due for your annual diabetic review. Please use the link to book your 20 minutes appointment with our practice nurse.

This review is for the benefit of patients with Type I or Type II Diabetes. 

Our initial Diabetic Annual Reviews are carried out face-to-face with a Practice Nurse and take 20 minutes. 

We ask patients to bring an early morning urine sample and if available a blood glucose diary with them to the review.

Urine specimen bottles are avilable for collection from the surgery's reception.

During your initial review, the Practice Nurse will;

• Take you height and weight
• Take your blood pressure
• Carry out an annual diabetic foot check.

Once your test result received our nurse will arrange for a telephone follow up to

• Review your HbA1c and cholesterol levels
• Review your blood glucose control
• Discuss any issues you have with your diabetes or health in general
• Advise any change in regimen, lifestyle or medication – including any side effects

Please note review appointments are bookable between 08:30 -11:00 Thursdays and Fridays.

Please make sure you drink plenty of liquid before your blood test.
